DVS-40 College Fee Waiver Application

  • Must be current revision – OCT 2019
  • New applications can be printed from:


Student’s Government-Issued Birth Certificate

  • Not acceptable: Abstracts, Ceremonial or Hospital Certificates
  • If seeking status as an Adopted Child or Stepchild, student must have entered into such status prior to 23rd birthday.


Proof of Student’s Income:

  • If student filed a tax return:
    • Submit student’s signed IRS 1040 income tax return        
    • If filed through E-File, we will need the IRS confirmation email attached with the E-File.


    • If student did not file a tax return, student must submit a Letter of Non-Filing from the IRS

Copy of the Veteran’s Award Letter or Summary of Benefits

  • Must include Veteran’s rating and effective date
  • Must be dated within the last year if applying for Plan A


Legal adoption paperwork if student was adopted



 Government-issued marriage certificate:

  • If seeking waiver as a Spouse or Step-Child
  • If veteran, spouse, or child has had a name change due to marriage.


Legal adoption order paperwork if student was adopted.            



If Student is applying under Plan A, then student must submit form VSD-020 indicating student is electing to opt-out of Chapter 35 educational benefits.



If the veteran is NOT at a 100% Rating, students will check the box NO on question “Are you receiving, or are you currently eligible to receive VA Educational Benefits under Chapter 35?” Only 100% rated veterans’ dependents are eligible for Chapter 35.



If veteran is deceased:

  • Death certificate is required
  • Form VSD-021 stating veteran is deceased with signature of non-veteran parent.
  • Non-veteran parent will sign the College Fee Waiver Application.
  • If both parents are deceased, then the student will sign the form.


No ERRORS on the application. If a mistake is made, put a single line through the error and write your initials. You must include a separate written statement clarifying each mistake.




  • College Fee Waiver application must be signed by both student and Veteran.
  • If the Veteran is not capable of signing, the non-veteran parent must sign the application and submit a written statement form (VSD-021) explaining why the veteran is unable to sign.
  • IRS Tax Forms must be signed by the student.


No questions can be left BLANK. If it does NOT apply to you please write N/A.



 Student's Annual Gross Income Limitation per academic year:

  • Academic Year 2018-2019 is $12,752.00
  • Academic Year 2019-2020 is $13,064.00
