What is the HCPCFC?

The Public Health Nurses in the Health Care Program for Children in Foster Care (HCPCFC), under the direction of the Fresno County CHDP Program, work with community programs and agencies to identify major obstacles faced by children in foster care in gaining access to coordinated, multidisciplinary services. The goals and objectives of the HCPCFC are to promote access to comprehensive preventive health and specialty services for children in foster care. HCPCFC nurses serve children within Fresno County foster care. The PHNs in the HCPCFC consult and collaborate with social workers and probation officers to meet the medical, dental, mental, and developmental needs of children and youth in foster care.

 

What services are provided?

  • Medical and preventive health care coordination and follow-up planning
  • Support to foster care providers in obtaining timely comprehensive health assessments and dental examinations
  • Expedited referrals for medical, dental, mental health, and developmental services
  • Coordination of health services for children in out-of-county and out-of-state placements
  • Medical care consultations and health consultations through the interpretation of medical reports
  • Training for foster care team members on the special health care needs of children and youth in foster care
  • Participation in the making and updating of the Health and Education Passport for every foster care child as required by law
