What is CHDP?

As a child grows, routine and complete physical examinations are important for good health. The CHDP Program provides free health exams for eligible children from birth to 21 years of age. CHDP can also help families find a doctor or dentist if they do not already have one.

Who does CHDP serve?

CHDP serves eligible children within Fresno County from birth to 21 years of age. Eligibility is primarily determined by age, but other factors that may be considered include:

  • If a child is in foster care
  • Family cost of health care
  • Well-child visits not able to be covered by insurance
  • Family income less than or equal to 266% of the federal poverty level

What services are provided by CHDP?

CHDP provides care coordination to assist families with medical appointment scheduling, transportation, and access to diagnostic and treatment services. 

How much does CHDP cost?

You can enroll in and receive services from CHDP for free.

How long can my child stay in CHDP?

Children can be enrolled as early as birth and enrollment within the program can continue until the child turns 21 years old.

How will CHDP help?

Children and youth enrolled in the program will receive periodic child health assessments and examinations, referrals to other providers as necessary, care coordination and assistance, and a certificate of health examination or waiver for school entry requirements.

How do I enroll my child in CHDP?

Applications for CHDP services are available at enrolled CHDP provider offices.
