Health Messages are written communication for Medical Providers, but can be informative for the general public. The health messages archived at the bottom of this page highlight important health and public health issues of which they should be aware.

Coronavirus Disease 2019 (COVID-19)

The US Centers for Disease Control and Prevention (CDC) has provided guidance for Healthcare Professionals for coronavirus disease 2019 (COVID-19). This guidance is based on what is currently known about the transmission and severity of COVID-19. The CDC will update this guidance as needed and as additional information becomes available. Reporting a Confirmed COVID-19 Case

Reporting Summary

  • Healthcare providers must report all Laboratory (PCR) confirmed COVID-19 cases and deaths to the DPH
  • Laboratories are required to report all positive and negative SARS-CoV-2 test results (PCR and serology)

Provider Reporting

Providers are required to report all laboratory confirmed cases of COVID-19 cases to the Local Health Department (per Title 17).

Suspected and Negative COVID-19 cases are not required to be reported at this time.

COVID-19 confirmed cases should be reported within 24 hours:

  • Electronically
    • Through the CalREDIE Provider Portal or eCR(preferred method)
      • More information about the Provider Portal can be found at CalREDIE webpage. Please fax applications to (559) 600-7607 Attn: Koch-Kumar.
    • Using an FCDPH online reporting form, available at
  • CMR Faxed
    • COVID-19 specific CMR form (confidential fax: 559-600-7607)
    • When reporting, information on patient demographics, signs and symptoms, testing, and exposure risk will be needed

In addition to the above reporting, a same day phone call during business hours (No after-hours calls required, cases outside of 8am-5pm can be reported the following day) to the LHD COVID-19 Provider Line at (559) 600-3332 is required for:

  • Patients who live or work in a congregated setting
  • Death of a COVID-19 patient
    • Deaths need to be reported regardless of if the patient was previously reported prior to death

Providers should also provide all febrile respiratory illness patients with an FRI packet, which includes important isolation/quarantine orders (see below).
