Rx for Health is a new referral system that will medical allow providers the opportunity to refer patients to community-based lifestyle change programs to improve and manage chronic health conditions. Rx for Health is made possible through collaboration between FCDPH, Cultiva la Salud and Clinica Sierra Vista.
Clinica Sierra Vista refers patients with high blood pressure to the Chronic Disease Self-Management Program (CDSMP) at Saint Agnes Medical Center. CDSMP is a six-week program that focuses on stress reduction techniques, exercise, and nutrition, in addition to using medication, evaluating treatments, and communicating with family, friends, and providers. This is a pilot program for Clinica Sierra Vista patients and other community members that is designed to lower patient rates of uncontrolled hypertension.
Clinica Sierra Vista also refers patients to the California Smokers' Helpline, which offers free smoking cessation counseling and resources.
The Rx (Prescription) for Health initiative is made possible with funding from the Centers for Disease Control and Prevention. The Fresno County Department of Public Health is working to reduce chronic disease prevalence and its associated health care costs.
