County-paid Policies 

All permanent County employees who are currently enrolled in a County health insurance plan are provided with a $10,000 Life and $10,000 Accidental Death & Dismemberment (AD&D) insurance policy at no charge to the employee. Management employees receive an additional $51,000 Life and AD&D insurance policies. Senior Management employees receive an additional $250,000 Life and $250,000 AD&D insurance policies. Please note that the additional Management and Senior Management policies are not contingent upon reenrollment in a County health insurance plan.

Employee-paid Policies 

In addition to the County-paid policies described above, all permanent County employees may purchase supplemental life insurance in the following amounts: $100,000 for the employee, $50,000 for their spouse, and $10,000 for their children.  The cost for these policies varies based on the age of the employee and whom they choose to cover.

Important Note: Employees who did not enroll during the initial open enrollment period may only enroll during a designated open enrollment period in the future, and their enrollment application is subject to approval by the carrier.  Employee Benefits will notify all employees of any future open enrollment periods.

Policy Certificate and Forms

  • Policy Certificate.  The Policy Certificate provides the terms and conditions of the County-paid Life and AD&D as well as the Employee-paid Life insurance.
  • County-paid Life Insurance Beneficiary Designation Form.  While the County-paid policies do not require an enrollment application, employees may use this form to designate their beneficiaries.
  • Employee-paid Life Insurance Enrollment Form.  Employees may use this form to terminate coverage, designate a beneficiary or elect coverage during a designated open enrollment period.
  • Employee-paid Life Insurance Portability Form.  Employees may use this form if they elect to continue their coverage if they are on an unpaid LOA or upon separation from service.

Waiver of Premium Benefit

Employees who are totally disabled for at least nine (9) months and on an unpaid leave of absence may be eligible for the Waiver of Premium Benefit.  In order to receive this benefit you will need to complete additional paperwork.  Please contact Employee Benefits at (559) 600-1810 or HRbenefits@fresnocountyca.gov for more information.

Other Coverage

Employees in bargaining units 1, 14, 35, and 37 as well as bargaining units 3, 4, 12, 22, 36 have access to life insurance policies sponsored by their bargaining unit.  Employees with questions regarding those policies are encouraged to contact their bargaining unit.
