All permanent County employees who are currently enrolled in a County health insurance plan are provided with a $10,000 Life and Accidental Death & Dismemberment (AD&D) insurance policy at no charge to the employee (no enrollment forms are necessary). Management employees receive an additional $51,000 life insurance policy and Senior Management employees receive an additional $250,000 life insurance policy.
Employees who are on an unpaid leave of absence and are totally disabled may elect to continue the policy by completing the forms below. Failure to complete these forms while out on an unpaid leave of absence may result in loss of coverage.
