Case Management Teams provide comprehensive, culturally & linguistically appropriate outpatient and community-based specialty mental health services to transition age youth, adults age 18 and older, & older adults age 60 and older that meet medical necessity. Services include assessments, clinical and rehabilitative services, case management, peer support and medication services within a wellness and recovery model.

Urgent Care Wellness Center

The Urgent Care Wellness Center strives to provide mental health treatment services that are client centered, strength based, culturally competent, and co-occurring mental health and substance abuse capable. Services are based in the Wellness and Recovery model as we believe that everyone can improve their health and wellness, live a self-directed life, and strive to reach their full potential.

Services provided are mental health assessment, client centered treatment planning, group therapy, limited individual therapy, crisis evaluation, and linkage and consultation with client support systems such as: primary care, psychiatric services, government agencies, private providers, and natural support systems such as family, friends, and faith communities.

Clinical Team

The Mission of the Adult System of Care (ASOC) is to provide culturally appropriate mental health and co-occurring services to the community of Fresno County. Utilizing a strength-based, wellness & recovery approach, services are provided by staff with a range of ethnic backgrounds and are culturally tailored to Fresno County's multi-cultural community. Services are provided in the office, in the community and at the client's residence. The ultimate goal of the Adult System of Care is to maximize the potential of each individual by providing a welcoming and compassionate environment while simultaneously including the client as an active participant in their own wellness & recovery. Further goals include emphasizing personal strengths, reducing barriers, enhancing adaptation skills and increasing meaningful activities. Also, ASOC instills hope and empowerment, personal responsibility, while reducing symptoms and developing broad-based community awareness.

Latino Team

The Mission of the Latino Team is to provide culturally appropriate mental health and co-occurring services to the Latino community of Fresno County.

The emphasis is on services provided by staff that represents the range of ethnic backgrounds and/or is culturally aware to the differences and similarities between these groups and the society at-large. Services are provided in the language of the clients. Services are provided in the office, community and at the client’s residence.

The ultimate goal of the Latino Team is to maximize the potential of each individual by focusing on their strengths with a “wellness & recovery” approach. We strive to include the client as an active participant in their own wellness & recovery, emphasizing personal strengths and individual recovery while reducing barriers, enhancing adaptation skills, increasing meaningful activities, fostering hope and empowerment, enhancing personal responsibility, engaging supportive relationships and value roles and reducing symptoms.

When possible, family involvement is a continuing goal of the Latino Team. The Latino culture is family-centered. The program works to involve family in services as appropriate. Due to the effects of their illness, many clients are alienated from their families and other significant persons, resulting in an increase in psychiatric symptoms, maladaptive behaviors, relapse of drug and alcohol abuse, low self-esteem, decreased functioning, social isolation, and hopelessness. Staff attempts to involve family and identified significant support persons in the treatment of the client.

Asian Pacific Islander Team

The Asian Pacific Islander Program (API) has provided culturally appropriate mental health and co-occurring services to Hmong, Cambodian, Laotian, Mien, Vietnamese, and other Asian descent residents of Fresno County since 1988. The emphasis is on services provided by staff that represents the range of ethnic backgrounds and/or is culturally aware to the differences and similarities between these groups and the society at-large. Services are provided in the language of the clients. Services are provided in the office, community and at the client's residence. The majority of clients are refugees suffering from war trauma and the complications of acculturating into a society vastly different from their own. Family involvement is a continuing goal for the individual treatment plan. Because the Asian Pacific Islander cultures are family-centered, the program works to involve family and significant others in the services, whenever possible. Due to the effects of their illness and potential acculturation problems, many clients are alienated from their families and communities, resulting in an increased in psychiatric symptoms and a decrease in functioning level. The API staff attempt to involve family in the treatment early on and to re-integrate clients in the life of their families and communities.

Pathways to Recovery

Pathways to Recovery Services support the recovery of women, men, and their children in our Substance Abuse Track, Therapeutic Children's Services and Mental Health Track. Services focus on treating all thinking, feeling, behavior, and/or substance use challenges that the client is experiencing. We use a client/child focused, strength-based wellness and recovery model as we walk with them. We know recovery is possible and that clients and their families can and do lead more productive and happy lives. It is never how challenging the thinking, feeling, behavior and/or substance use is, it is rather how we function in our world as we address them.

Older Adult Mental Health Clinic (Serves Ages 60 and Older)

The Older Adult Behavioral Health Team consists of Psychiatrists, Nurses, Mental Health Clinicians, Community Mental Health Specialists (CMHS), Mental Health Workers and Peer Support Specialists (PSS) who provide behavioral health services to adults at or near age 60 years old and older who have a significant impairment due to mental illness and can benefit from services which include a comprehensive behavioral health assessment and treatment plan; individual, family and group psychotherapy; individual and group rehabilitation; case management; collateral; crisis intervention and assistance with access to psychiatric medication services. The team works collaboratively with several nearby Department of Social Services agencies and programs, including Adult Protective Services and In-Home Supportive Services, as well as the County Ombudsman and Physical Healthcare providers and clinics throughout the greater metro area of Fresno, to improve services to the seniors in our communities. Team Clinicians and CMHS staff also work closely with Team PSS staff in providing strategic outreach and education to the community. The Older Adult Team supports the wellness, recovery and individual perspectives of each person who receives our services and strives to create a stimulating and creative environment that supports the growth and development of each client, family member and community partner with whom we come into contact as well as each of our staff, administration and supporters.

Perinatal Wellness Center

Mission Statement: Strengthening and healing families by instilling hope, providing advocacy and support, and promoting health and wellness during pregnancy and postpartum.

Service: The Perinatal Wellness Center provides mental health services to pregnant and postpartum mothers and their babies, as well as to fathers struggling with paternal postnatal depression. The multidisciplinary team is trained and certified in Perinatal Mood and Anxiety Disorders and includes Mental Health Clinicians, Infant mental Health Specialists, Case Managers, Psychiatrist, Nurse Practitioner, LVN, Public Health Nurses, and Peer Support Specialist. Clients may be referred from their medical provider or may self-refer.
