 What is the Nurse-Family Partnership Program?

The Nurse-Family Partnership is a special program for first time mothers and their child. A Public Health Nurse visits the pregnant women in her home during her pregnancy and until her child is two years old.

How does a women get enrolled in the program?

Women are referred by their doctors, school nurses, Fresno County outreach workers, public health nurses or by self-referral.

Who does the program serve? The Nurse-Family Partnership serves low income, first time mothers in Fresno County.

What services do you provide?  Education on pregnancy, labor and delivery, breastfeeding, infant care, discipline, and growth and development of your baby and toddler. Plus, we provide information on safety, returning to school, jobs and a variety of other topics.

Why should I join? The program supports and encourages young women to be the best that they can be, for both their child and themselves.  

What does the program cost?The Nurse-Family Partnership Program is free.  

 

How will the Nurse Home Visitor be able to help me?  The Nurse Home Visitor supports and encourages women to set and obtain their own life goals. They provide information and community resources. They are advocates for the young women in the program.  

How long does the program last?  The nurse will visit you during your pregnancy (before your 27th week) and until your baby is two years old.  

How long are the visits?  Each visit lasts from 1 to 1 ½ hours.
